Measuring Health Equity and Disparities in Practice and Policy

Identifying health disparities and inequities is a top concern among health care organizations across the country, especially as mandated reporting increases in areas such as value-based care arrangements. And while these new requirements bring an increased emphasis on understanding health equity, many organizations don’t know how or where to start … the ACG System can help.

To learn why the ACG System is essential for organizations looking to develop an accurate and beneficial plan to combat health disparities, download our Health Equity e-guide “Disparities in Health Care and How Population Health Analytics Can Help.

The Landscape

Over the last several years, evaluating health equity metrics has become a critical focus. Federal Medicare payment systems are rolling out health equity measures in their agreements, as part of the plan to close the gap in health care access among disadvantaged populations. Other organizations are following suit.

Submitting the necessary metrics to these groups will require health care providers to accurately measure health equity in their own communities. Without the benefit of advanced software, providers and health care organizations are left to interpret the data on their own. The Johns Hopkins ACG System allows users to take patient data they already have and develop the analytics to create an intelligent health equity strategy that also meets Federal reporting guidelines.

Analyzing the Population

To understand health equity and disparities in a population, the population must be examined and measured appropriately. By adding segmentation and stratification, this ensures each population receives an appropriate level of care.

Using measurements such as race, ethnicity and disability status, the Johns Hopkins ACG System analyzes a population to view their most immediate needs. Data that was once hidden is now able to be shared widely. The result is a strong population health strategy that benefits the community as a whole. Local organizations can construct targeted programs that create immediate benefits for underserved individuals. These methods provide a thorough approach to the analytics necessary for a well-developed health equity strategy.
