Prioritize Resources and Deliver Higher Quality Care

The Johns Hopkins ACG System is the premier population health analytics tool used by providers, ACO’s, health systems and health ministries worldwide. The ACG System’s suite of tools are used to measure health outcomes, support cost of care reduction activities, and case mix adjust for population characteristics. Our world-class predictive models accurately predict total cost, admissions and readmissions.

The ACG System helps providers easily identify patient sub-populations with distinct clinical needs, to help focus resources where they are most needed. Providers can prioritize patients for preventive care outreach initiatives that help maintain patients’ health. This case study shows an example of how one of our customers used the system to support their urgent COVID-19-related patient needs.

CASE STUDY: MARATHON HEALTH

Marathon Health saved its clinics $22.5 million. The team created a customized algorithm using ACG System risk markers including age, gender, chronic conditions, probability of hospitalization, and predicted costs. They were able to identify – and provide prioritized care for – those individuals most at-risk for complications from COVID-19.
