As our population ages and the prevalence of long terms conditions increases, our need to coordinate and integrate the care of the most vulnerable and at risk people in our communities grows in importance. A risk stratification methodology which recognizes multimorbidity, classifies across the whole population and allows users to identify patients at all levels of the risk pyramid is a key tool in the identification of people who will benefit from the coordination of their care.
