As our population ages and the prevalence of long terms conditions increases, our need to coordinate and integrate the care of the most vulnerable and at risk people in our communities grows in importance. A risk stratification methodology which recognizes multimorbidity, classifies across the whole population and allows users to identify patients at all levels of the risk pyramid is a key tool in the identification of people who will benefit from the coordination of their care.

Learn about:
  • The latest policies and innovative thinking in relation to integrated care
  • How the application of the ACG System and its predictive models can support the early identification of individuals who could benefit from coordination and integration of their care
  • How CCGs are using the ACG System to profile their populations and gain a better understanding of the prevalence of long term conditions and multi-morbidity and how this information is informing commissioning decisions
  • The experience of other parts of the world
