This award-winning document presented at the 2015 PCSI conference highlights work undertaken with Slough Clinical Commissioning Group in the UK. The work has challenged some of the preconceptions about what factors drive high cost and high utilisation in their population and argues that in order to undertake effective case finding you first need to profile of the population you are managing. You then need to use sophisticated case finding techniques to align the care programs available to the needs of the population and then continue to iterate between population profiling ands case finding.
