COVID-19: Proactive High Risk Patient Outreach in the UK

Located in the UK, NHS South Central & West Commissioning Support Unit (SCW) are long-term users of the ACG System. When the UK Government and National Health Service required the identification of individuals at a higher risk of complications from COVID-19, SCW knew it would require the proactive risk management of 20% of the population.

Using data from the Johns Hopkins ACG System, SCW segmented and stratified the at-risk groups so that they could prioritize candidates for proactive clinician, care manager and social worker intervention.
